	Click here to view/download the CESSWI Application Packet in Adobe Acrobat (pdf) format. If paying the application fee by credit card, print and fill-out the credit card form included in the packet, and send it with your application.
	Electronically Submitting an Application If paying the application fee by credit card, you can send your completed application by email to: If you send your application electronically, you should receive a receipt for the application fee within one week (7 days). If you don't get the receipt, please follow-up with us to make sure we actually received your application. Do NOT follow-up an electronic application with a hard copy; your credit card will be double-billed.
